Charge/Admissions Nurse
Job Summary:
The Admissions Nurse is responsible for coordinating and overseeing the admission process for new residents in a skilled nursing facility (SNF). This role ensures a smooth transition for patients by conducting initial assessments, verifying medical information, and collaborating with interdisciplinary teams to provide quality care from the moment of admission.
Key Responsibilities:
- Admissions Process Coordination:
- Conducts initial nursing assessments to determine patient needs and appropriate level of care.

- Ensures all required admission paperwork is completed accurately and in compliance with state and federal regulations.
- Coordinates with hospital discharge planners, case managers, and family members to facilitate smooth transitions.
- Clinical Assessment & Documentation:
- Performs detailed physical assessments upon admission.
- Documents patient history, current health status, and any special care requirements.
- Communicates patient needs to the care team, including nursing, therapy, and dietary staff.
- Resident & Family Support:
- Educates patients and families on facility policies, care plans, and available services.
- Addresses concerns and ensures a positive experience during the admission process.
- Compliance & Quality Assurance:
- Ensures all admissions comply with Medicare, Medicaid, and private insurance guidelines.
- Maintains accurate and up-to-date records following facility policies and HIPAA regulations.
- Identifies potential risks or barriers to admission and works to resolve them.
Qualifications & Skills:
- Active RN or LVN/LPN license.
- Experience in admissions, case management, or skilled nursing preferred.
- Strong assessment, documentation, and organizational skills.
- Knowledge of Medicare, Medicaid, and insurance reimbursement guidelines.
- Ability to communicate effectively with healthcare professionals, residents, and families.
- Proficiency in electronic medical records (EMR) and PCC systems.
Work Environment:
- Primarily within the skilled nursing facility, with possible travel to hospitals for assessments.
- Fast-paced environment requiring multitasking and problem-solving.
- Interaction with a variety of healthcare professionals and residents with diverse needs.
